J-1 Scholar or Student Intern Departure

For record-keeping and immigration reporting purposes, International Student & Scholar Services needs to know when the J-1 Scholar or Student Intern leaves the University of Colorado Denver | Anschutz Medical Campus or completes J-1 status.

Complete the J-1 Scholar or Student Intern Departure Form within 2-3 weeks prior to the scholar's departure. The form should be completed by the HR Business Partner or the contact for the department.

After submission of the form, short surveys will be sent to faculty host and J-1 scholar or student intern about their experience during the J-1 program at the University of Colorado Denver | Anschutz Medical Campus. ISSS uses the information to evaluate the J program and to report to the U.S. Department of State as required by federal regulation.
